10/7/2023 EnchantaVaughn Approved Behavior: QMHP-A arrived for the session and greeted Ms. Cosby. QMHP-A assessed Ms. Cosby’s current stress/anxiety levels. QMHP-A inquired about her day. She verbalized to QMHP-A her day was going well. Ms. Cosby did not present any symptoms of S/I or H/I at the time of the session. Intervention: QMHP-A assisted Ms. Cosby in increasing her time daily by involving herself in more relaxing activities and exercises. QMHP-A explored Ms. Cosby’s pattern of intolerant, impatient interaction with other people. QMHP-A assisted Ms. Cosby in recognizing and understanding how when she became impatient and intolerant of other people it triggered her angered response. QMHP-A assisted Ms. Cosby in recognizing the importance of learning to control this response. QMHP-A utilized modeled behaviors to educate Ms. Cosby in applying assertive communication, with emphasis on recognizing and refraining from aggression ignoring the rights and responsibilities of others as it relates to her own wants, needs, or demands. QMHP-A utilized modeled behaviors to assist Ms. Cosby in decreasing her impatience with others by talking of appreciating and understanding the good qualities in others. Response: Ms. Cosby admitted that she had a low tolerance for people and did not like to wait for things. Ms. Cosby exhibited a communicative character during the session evident by her open manner as she spoke. Ms. Cosby shared past and recent events that involved her being short tempered with people and impatient because of whatever reason. Plan: QMHP-A will teach the client relaxation skills (e.g., progressive muscle, guided imagery, slow diaphragmatic breathing) and how to discriminate better between relaxation and tension; teach the client how to apply these skills to her daily life. (1-2x weekly).
